Vertigo is the sensation that you or your surroundings are spinning. The most common cause is BPPV (loose crystals in the inner ear). Other causes include vestibular neuritis and Meniere's disease.
BPPV is caused by loose calcium crystals in the semicircular canals of the inner ear. It causes brief spinning triggered by head movements and is treated with the Epley manoeuvre.
The Epley manoeuvre — a series of head repositioning movements performed by the doctor — resolves BPPV in most patients after 1–3 sessions. No medication is usually needed.
Seek emergency care if vertigo comes with sudden severe headache, diplopia, facial weakness, slurred speech, or severe unsteadiness — these may indicate a stroke.
Betahistine, cinnarizine, and prochlorperazine help acute episodes. Long-term medication is minimised to allow the brain to compensate naturally through vestibular rehabilitation.
