High-risk pregnancy involves conditions increasing risk to mother or baby — advanced maternal age, twins, GDM, pre-eclampsia, previous C-section, placenta praevia, or pre-existing medical conditions.

Pre-eclampsia is high blood pressure (>140/90) with proteinuria after 20 weeks. It can progress to eclampsia (seizures) and requires hospitalisation and often early delivery.

GDM is high blood sugar first detected during pregnancy. Managed with diet, exercise, and if needed insulin or metformin. Uncontrolled GDM increases risk of large baby and C-section.

Based on foetal position, estimated weight, previous surgical history, placenta location, maternal pelvis, and intrapartum foetal condition. Birth plan discussed at 36+ weeks.

6-week postnatal check, wound care, breastfeeding support, iron treatment, contraception counselling, and postnatal depression screening.
