Overview

Ovarian cysts are fluid-filled sacs that develop on or within the ovary. Most are benign functional cysts that resolve spontaneously. However, some require treatment - particularly large cysts, endometriomas, dermoids or cysts with concerning ultrasound features.

Ovarian cysts are fluid-filled sacs within or on the surface of an ovary. Most are functional (related to the menstrual cycle) and resolve naturally within 1–3 menstrual cycles.

Many cysts are asymptomatic. Symptoms when present: pelvic pain, bloating, irregular periods, pain during intercourse, and urinary frequency.

Treatment when a cyst causes severe symptoms, is >5–7 cm, has complex features, persists >3 months, or ruptures/twists (ovarian torsion — a surgical emergency).

Most functional cysts do not affect fertility. However, endometriomas and PCOS can impair ovulation and require treatment to optimise fertility outcomes.

Ovarian torsion is a gynaecological emergency where the ovary twists around its blood supply causing sudden severe pain, nausea, and vomiting. Requires urgent surgery to preserve the ovary.
