Common causes: endometriosis, ovarian cysts, fibroids, pelvic inflammatory disease (PID), adenomyosis, interstitial cystitis, irritable bowel syndrome, and musculoskeletal causes.
PID is infection of the uterus, fallopian tubes, and/or ovaries usually from sexually transmitted bacteria (chlamydia, gonorrhoea). Causes fever, lower abdominal pain, and purulent discharge.
Seek urgent care for: sudden severe pain (suspected ovarian torsion or ectopic pregnancy), fever with pelvic pain (PID), or missed period with pain and positive pregnancy test.
Many cases respond to NSAIDs, hormonal treatment, physiotherapy, nerve blocks. Surgery is reserved for specific diagnoses like endometriosis or ovarian cysts.
